OTO after paying for a primary product can only currently be added with a classic checkout pages, so for offers sold as paid appoints: you can't add any upsells if they paid through the event scheduler that has the nice calendar (the one set up in the appointments app under event types).

The paid event booking calendar needs it's own editable page where checkout can be integrated so that OTO can be done with paid appointments.
