Add the ability to create a custom, fillable form with check boxes and writeable text for a client to choose options etc. Would like this to be accessible by both the client and me so the client can edit it when needed and I can view it when needed.

Hugh (Product Support Manager) I attached a screenshot of a portion of a form we use to collect choices from our clients (we run a hunting membership that involves choosing the species they want to put in for a draw (lottery system) every year. We need to collect information like this when they sign up and then each year get it updated. It would be great if we had a way to easily collect this where it was visible to us on the backend but also seen by the client so they can reference it in their account and update if needed throughout the year if they change their mind.
We also need the ability to collect other personal information like address, phone, email etc which looks to already be available in the system.
